Dr Patricia Kailis Award Criteria: This award celebrates achievement in the areas of innovation and entrepreneurship for alumni aged 40 years and younger. There are no restrictions on who may propose nominations. 2. Nominees must have completed their senior schooling at PLC Perth (although they may have left earlier than Year 12 or equivalent final year). 3. An Old Collegian cannot win more than one award category in any calendar year and cannot win the same award category more than once. 4. Awards will not be awarded posthumously. 5. Joint category nominations will not be accepted. Separate nomination forms for each category are required. 6. Old Collegians may nominate themselves or be nominated by another person. 7. Current members of the PLC Perth Council or the PLC Foundation are not eligible to be nominated. 8. The judging panel’s decision is final on all matters relating to the nomination process and the selection of winners and no correspondence will be entered into. 9. The judging panel reserves the right to allow nominations to remain open for up to two years. Nominators will be responsible for updating additional information. 10. The judging panel reserves the right not to allocate an award in one of the categories if, in its members’ opinion, there are no entries of sufficient merit. The number of awards allocated in each category is at the judging panel’s discretion. 11. If consensus is not reached by the judging panel, the final decision will be made by the Principal. 12. The School reserves the right to disclose the identity of the nominator to the nominee. 13. Personal information submitted to the School by the nominator or nominee, in connection with the awards, will be provided to the judging panel for the purpose of judging the awards. 14. To avoid disappointment and to respect the privacy of those consulted, the nominations are confidential, and we ask that nominators and others involved maintain confidentiality. 15. The nominator’s and / or winning nominee’s name, image and statement may be published and/or used in any form of media, in any context pertaining to the awards. 16. Winners and their nominators, and all other nominators, will be contacted personally regarding the outcome of their nomination. 17. Winners will be invited each to the PLC Perth Speech Night, where the Award recipients will be announced.
